Forms - Detailed Explanation of Fields

List View

  • Find: Begin typing the name of the content you would like to find and the system will perform a search. NOTE: Do not click the enter key as you type. This will remove the Search terms.
  • Sort Function: When scrolling over the top of the list, the Heading turns dark and an Arrow appears next to the Heading. Once you see the Arrow, simply click the Heading to Sort the List in alpha-numeric order. Click again for a reverse order of the list.
  • Edit Selected: To change the Publish Status or to Delete selected items, click the check boxes to the left of each item. When the boxes are checked, the "Edit Selected" button will turn on next to the Add button. Click "Edit Selected" to change Status or Delete.
  • Edit Content: To edit your content, click the underlined words that are in a different color. When scrolling over the words, a pencil icon will appear and the row will change to a light orange.

Add a New Form

Note: This information must be filled out before creating form fields.

  • Name: The name of the form, this will be displayed on the front-end of your site if this is a form displayed on your site.
  • Form Type -- Display Form on Website: this form will be displayed on a page on your website.
    • Description: The description will appear before the form.
    • Submit Button Image: Select an image to use as the submit button of the form.
    • Results Page: After the form is submitted the user will be taken to this page. If 'Same Page' is used, the form will be replace by thank you text.
    • Submissions: This checkbox will allow only one form submission per member. Accessing this form will require login.
    • E-mail: When form is submitted, send form submission to this email address. To designate multiple recipients, separate them with a comma.
  • Form Type -- Display Form during Event RSVP: this form will be displayed to users RSVP'ing for an event.
    • Event RSVP: indicate which event for this form to show, or indicate all events
    • Request Information for: form will be presented to the selected user groups
    • E-mail: When form is submitted, send form submission to this email address. To designate multiple recipients, separate them with a comma.
  • Form Type -- Display Form during Product Purchase Checkout: this form will be displayed to during a purchase of a specified product
    • Product Families: indicate which product family for this form to show, or to show for all product families
    • E-mail: When form is submitted, send form submission to this email address. To designate multiple recipients, separate them with a comma.

Add Form Fields

 Add a block of text around form fields.

Add Form Element

  • Label: Short Description of what users should enter.
  • Description: Longer Description of what users should enter.
  • Attr: Attributes of the field. The field can be validated that data was entered or that data is an email address.
  • Type: Choose the type of date to be collected.
    • Text Field: A few words, less than 255 characters.
    • Text Area: Allows for a longer response.
    • Drop Down Menu: Allows users to select one option from many choices. After saving field, each option is entered.
    • Check Boxes: Allows users to select many options from many choices. After saving field, each option is entered.
    • Radio Buttons: Allows users to select one option from a few choices. After saving field, each option is entered.
    • Captcha Security Input: Requires users to enter a code to ensure that spam does not get entered to the form.

View Form Values

On this screen, you can view the values that users have submitted for this form. It functions much like other list screens in the system, allowing you to filter based on Month/Year of submission, search, and sort. You can also export the list of submissions using the "Securely export this report" button.

Edit a Form

Clicking the name of any form on the form list screen will allow you to edit your form. 

Copy a Form

You can create a copy of your form from the edit screen by clicking the "Copy Form" button in the sidebar.
